By supplying your contact information, you authorize Diamond Cutter Wisdom Pte Ltd (its staff and authorized personnel) to contact you with further information. You agree that Diamond Cutter Wisdom Pte Ltd may collect, use and disclose your personal data, which you have provided in this form, for providing you with timely and relevant information and marketing material, in accordance with the Personal Data Protection Act.

By filling this form, you have given your consent to the following:

I hereby consent to receive information sent by our organization about our organization’s products and services, follow-up on registration and any of your involvement for our events, including updates on our latest promotions and new products, services, via the following channels:

  •  Email 
  •  Text Message
  •  Phone Call

 

I hereby give my consent to Diamond Cutter Wisdom Pte Ltd to photograph, film, and then use, reproduce, and publish said images of me.

  • I agree that photographs or film thereof shall constitute the sole property of Diamond Cutter Wisdom Pte Ltd, with full right of disposition in any manner whatsoever, including the right to publish on Facebook, etc.
  • I hereby release Diamond Cutter Wisdom Pte Ltd and his/her legal representatives and assigns from any and all claims whatsoever in connection with the use, reproduction, publication of the images thereof.

 

Refund | Exchange | Transfer Policy

  • The transfer is only limited to registering for the same workshop or city intensive. The transferor is solely responsible for the transfer and inform the person in charge of DCW after the transfer is successful.
  • Every e-confirmation (with serial number) is valid for only one participant
  • An administrative charge of 25% is imposed for cancellation 3rd week (day 14-21) before the event.
  • An administrative charge of 50% is imposed for cancellation 2nd week (day 7-14) before the event.
  • An administrative charge of S$50 is imposed for transfer 2nd week (day 7-14) before the event.
  • Exchange or refund or transfer is not permitted for the last week (day 1-7) before the event
  • Exchange or transfer is not permitted for Follow-Up Program
  • Diamond Cutter Wisdom (DCW), Global Diamond Cutter (GDC) & Diamond Cutter Institute Global (DCIG) will not be liable for invalid entry arising from misuse or loss of e-confirmation with serial number or attempt at multiple participants entry
  • Diamond Cutter Wisdom (DCW), Global Diamond Cutter (GDC) & Diamond Cutter Institute Global (DCI Global) reserve the right to amend the refund, exchange & transfer policy at any time
  • The deposit amount is non-refundable
  • Workshop Information will be mailed to you 2-4 weeks prior to the event. (Pls provide CLEAR email /mailing address)

PERSONAL DATA PROTECTION ACT 个人资料保护法 (DCW & GDC)

CONSENT FORM 同意书 – DIAMOND CUTTER WISDOM & GLOBAL DIAMOND CUTTER

1. In compliance with the Personal Data Protection Act (“PDPA”), The Diamond Cutter Wisdom & Global Diamond Cutter (“DCW & GDC”) seeks your consent to collect and use your personal data (i.e. Name, NRIC, contact numbers, mailing and email addresses) in order to maintain the DCW & GDC registry of students and to disclose such personal data to DCW & GDC’s affiliated organizations where necessary (“Purpose”).

2. DCW & GDC will also collect and use your personal data to provide you with information on studentship, membership and activities such as upcoming events, seminars, workshops, conferences and training programmes organized by DCW & GDC and its affiliated organizations which may be relevant to you (“Services”).

3. DCW & GDC respects your privacy and assures that your personal data will be kept securely according to PDPA.

4. I hereby give my acknowledgement and consent to DCW & GDC to use my personal data for the aforesaid Purposes and Services. In the event that I have registered my Singapore / Malaysia telephone numbers(s) with the Do Not Call Registry and wish to withhold or withdraw my consent to DCW & GDC in respect of receiving telephone calls and/or SMS, I endeavour to provide sufficient notice to DCW & GDC of such as soon as reasonably practicable. I further agree to indemnify DCW & GDC against any financial penalties imposed by the Personal Data Protection Commission or any court of law in Singapore/Malaysia as a direct or indirect result of my failure to inform DCW & GDC of my registration with the Do Not Call Registry.

5. I agree that my consent will remain in place until my withdrawal by officially notifying DCW & GDC in writing at admin@diamondcutterwisdom.com & admin@globaldiamondcutter.com
